SEL360 Family Program Selection

Your child’s school provides SEL360, a school-based social-emotional learning program, to students during the school day. SEL360 also connects families, on an optional basis, to SEL360+, additional individual clinical and mental fitness support delivered by the school’s licensed clinical partner and billed to your family’s private and public insurance, primarily Medicaid. This form lets you choose your child’s level of participation in each. Please complete one form per student.
