mySHO + Scalable Care Mental Health Research as a Service Platform (MHRaaS)

 Overview

MHRaaS leverages conversational AI and advanced automation to help researchers and clinicians significantly scale the delivery of digital health interventions.

Our MHRaaS platform streamlines the design, management, and execution of digital health programs for research and removes the technical hurdles that typically block a promising new application from becoming a real option for clinical practice.

This platform is particularly well suited for mental and behavioral health interventions but can be expanded to support a variety of digital health applications.

GritX is an example of the MHRaaS platform, and was developed in collaboration with the University of California San Francisco's Langley Porter Institute

Benefits for Research:

●      Explore new ground in treatment research

●      Reduced Costs and Effort: Ideate and design programs within the platform, provide previews to peers for timely feedback, and export the protocol information for ease of inclusion in IRB documents.

●      Streamlined Delivery: Automate recruitment, assessment, and intervention delivery, saving time and resources.

●      Enhanced Engagement and Retention: Utilize AI-powered chatbots and personalized interventions to keep participants motivated and on track.

●      Simplified Data Collection and Analysis: Seamlessly gather health data through web and mobile apps, and export it for analysis.

●      Improved Research Quality: Maintain rigorous scientific standards with secure data storage, compliance tools, and real-time monitoring.

Benefits for Clinical Use:

●      Reduce Time to Market: Once a program is approved for clinical or commercial use, the app workflows can be adjusted as needed in the MHRaaS platform and simply released to market without new infrastructure or major development work.

Benefits for Patient Use:

●      Convenience: Access the program and complete tasks via web or mobile app, maximizing opportunities for intervention and minimizing disruption.

●      Personalized Support: Receive AI-driven guidance and encouragement through the chatbot, tailored to participant needs and progress.

●      Contribution to Advancements: Make a valuable impact on medical research and future treatment options.

MHRaaS Care Capabilities:

Patient Experience:

●      AI Chatbot: Patients engage with an AI chatbot that uses natural language processing to respond in context and initiate actions - like presenting relevant questions or content - based on the conditions defined for the program.

●      Content Library: A collection of content - articles, videos, exercises, or other interactive or learning material - is readily available for patients to browse, bookmark, and engage with on demand.

●      Xpeditions: Patients may choose to engage in longer, curated journeys that leverage both chatbot capabilities, as well as a selection of content pre-configured for the topic. Xpeditions are intended to be more interactive and dynamic experiences for the participants and are available on demand, much like items in the content library.

●      Web & Mobile Delivery: When approved, the program can be made available to patients as white-labeled web and mobile applications, tailored to match the branding specific to your organization or audience.

●      Multilingual Support: The platform can support diverse populations with language-agnostic capabilities.

Researcher and Clinician Onboarding:

●      Program ideation and design collaboration: Whether converting an existing intervention to an automated program or designing a new study from scratch, MHRaaS will offer insights and best practices for using new and innovative options offered by AI and other proven digital platform technologies.

●      Initial program configuration: MHRaaS provides the “first draft” of a program in a sandbox environment where researchers and clinicians can create original content and configure bot interactions until their program is ready to release.

●      Custom design: MHRaaS will provide custom branding and design elements to the digital program so that it matches the style of the brand or organization it represents.

●      Custom integrations: If a program requires data from an external system or biometrics from a device, MHRaaS will deliver the necessary integrations and leverage that data as appropriate in chat bot interactions and data output.

●      Training and support: Experts will educate researchers and clinicians about how to best use AI chatbots for interventions, as well as train them on how to modify the various interactions in the configuration. Researchers and clinicians will get hands-on support from inception to publication of the digital health program.

Program Configuration Options:

●      Configurable Knowledge Base for AI chatbot: The AI-powered chatbot provides personalized engagement through natural language conversations, prompting participants to engage with contents, as defined by the program design.

The Knowledge Base forms the core of the bot's capabilities, enabling it to interact intelligently and contextually with users, manage conversations effectively, and deliver personalized content and responses. It's a comprehensive set of tools and functions that support the creation, management, and execution of sophisticated conversational flows.

Create and configure the items that power the chatbot's ability to understand, interact, and respond to user inputs:

○      Actions are the operations that the bot performs in response to user inputs or changes in the dialogue state. This includes sending messages, changing dialogue states, starting new dialogues, submitting forms, asking for parameter values, setting parameter values, sending content to users, not repeating sent content, searching for content, and requesting feedback after sending content. Actions are crucial for the bot to interact dynamically with users and manage conversation flows.

○      Questionnaires are structured forms that can be used within dialogues to collect user inputs systematically. They are used for surveys, assessments, or other data collection purposes. Questionnaires allow for a structured interaction where users can provide information in a predefined format.

○      Utterances are messages that the bot sends to users. They can be crafted to respond to specific user inputs or to guide users through a conversation. Utterances can be dynamic and may include templates with parameters to personalize the messages based on the user's language, inputs, or other contextual factors.

○      Conditions define the rules for triggering actions or otherwise delivering content. They help make the bot's responses context-aware and relevant to the current state of the conversation or the user's inputs.

○      Parameters are variables or data points that the bot uses to understand and manage the conversation's context. They can include user-specific details, session variables, or other relevant data that the bot needs to process user inputs effectively and deliver personalized responses.

○      Content Delivery functionality manages and delivers content to users during conversations. It includes features for organizing, selecting, and sending content based on the conversation's context, user preferences, or predefined rules, as described above.

●      Content Management: The platform provides the ability to create and manage sets of content, like articles, images, blog posts, exercises, or other guided activities.  Contents may be presented to users in the context of a regular chatbot interaction, available as part of a more focused Xpedition, or accessible as stand-alone items in the user’s Library.

●      Program preview: Ideate, create, and modify the program in a safe, closed sandbox environment, allowing you to gather feedback from designated collaborators prior to releasing to real participants.

●      Export Protocol: Once the program content and workflows are finalized, export all of the content as the “trial protocol” for inclusion in IRB documentation, further shortening the path to approval.

Participant Recruitment and Engagement:

●      Multi-channel recruitment: Engage potential participants through public platforms, existing health systems, or targeted campaigns.

●      Screening and enrollment tools: Ensure eligibility and collect baseline data efficiently using standardized or trial-specific questionnaires.

●      Randomization and stratification: Maintain scientific rigor through automated participant allocation.

●      Participant engagement: Send invitations, notifications, reminders, and updates via automated emails, texts, push notifications, or integration into 3rd party messaging apps.

●      EMR/EHR Integration: Integrate participant health records for use with recruitment or personalization of interactions and interventions.

Health Data Collection, Monitoring, and Analysis:

●      App data capture: Track self-reported data, biometrics, and sensor readings through user-friendly web and mobile apps.

●      Device integrations: Leverage data from Android, iPhone and other wearables for comprehensive data collection.

●      User engagement monitoring: Track participant progress and trigger engagement reminders as needed.

●      Supported data analysis tools: At the end of the trial, export data for seamless integration with preferred analytical platforms like R and SAS.

●      Adverse Event Handling: Monitor assessments and bot interactions to detect adverse events in real-time for interventions and reporting during the trial.

Security and Compliance:

●      HIPAA-compliant participant data access and storage: Ensure participant privacy and data security with robust security protocols.

●      Audit logs and tracking: Maintain transparency and accountability throughout the research process.

●      Regulatory support: Navigate ethical and regulatory requirements with expert guidance and resources.
